FIRST VISIT

Within our private practice model, your therapist is your primary contact, so you will not need to check in with a receptionist upon arrival. Please take a seat in our waiting area and your therapist will greet you for your appointment. When receiving Telehealth, your therapist will meet you in the patient portal at the time of your appointment. Please submit the following REQUIRED items prior to your intake appointment:

  • Completed Client PAPERWORK ON THE CLIENT PORTAL INCLUDING:

    • CLIENT HISTORY FORM​

    • CLIENT INFORMATION FORM

    • CLIENT INSURANCE FORM

    • EMERGENCY CONTACT FORM

    • INSURANCE FORM/PAYMENT AUTHORIZATION FORM

    • Signed Consent for SERVICES form

    • SIGNED NOTICE OF PRIVACY PRACTICES

    • Completed Release of Information form (if desired)

  • Photo ID (of legal guardian, if client is a minor)

  • Payment for copay or other financial responsibility (cash, check, credit/debit card)
